PARENTS’ COMMITMENT & ADMISSION AGREEMENT
THIS AGREEMENT IS BETWEEN Centerpointe Christian Preschool (THE PRESCHOOL) AND
_______________________________(parent/guardian) for the care of________________________________.
1. We have read and understand the school’s vision, mission and educational philosophy and we agree to actively support the purpose and philosophy of Centerpointe Christian Preschool.
2. We understand that mutual care, commitment to honest and respectful dialogue, and conduct consistent with Biblical standards is necessary for a healthy preschool family environment, and we agree to abide by and support these values and the established policies of the school.
3. We understand that positive parent involvement is a significant factor in the success of the preschool program and we agree to support the school through our involvement in Parent-Teacher Conferences, Open House, Parent-Teacher activities, and other school-sponsored meetings and activities.
4. We understand that the State of California Licensing Agency (Community Care Licensing) or Child Protective Services has the following authority:
a. To interview child(ren) or staff, and to inspect and audit child or facility records without prior consent;
b. To observe the physical condition of the child(ren), including conditions which could indicate abuse, neglect or
inappropriate placement and to have a licensed medical professional physically examine the child(ren).
5. We understand the school’s legal obligation, as mandated reporters, to communicate occurances/incidents of suspected child abuse to the appropriate authorities.
6. We give our consent to Centerpointe Presbyterian Church and Centerpointe Christian Preschool to include photograph(s) and/or video(s) of our child(ren) in its promotional materials or its website. (Never using name, birthdate or any specific information.) ______________
7. We understand and will fulfill our financial commitment to pay for the educational services the school is providing for our child(ren).
a. Daily program services covered by monthly tuition include a qualified staff to supervise activities that assist your child’s learning process. This includes varied activities of play, reading readiness, music appreciation, stories, creative art, science, health, all of which provide for an educational experience of lasting value in the physical, mental, and emotional development of the child(ren). The teaching of Jesus will be shared throughout the entire curriculum. A mid-morning snack and afternoon snack will be served daily.
8. We agree that we are contracting for the services which are marked below and will pay the stipulated rates indicated. We also understand that tuition rates may be raised annually to maintain the quality programs we expect for our children.
a. A non-refundable Registration Fee of $200 is due at the time of registration for new students, and due at the time of re-enrollment for returning students or students who have previously withdrawn from the program.
b. An annual supply fee of $100 per child is due at the time of Registration.

Tuition not paid by 5:00 pm on the tenth of the month is delinquent and will be assessed a late fee of $35. Parents whose tuition becomes more than thirty days will not be allowed to send their children to school until tuition is brought current or arrangements to pay are made through the office. No credit on tuition is given for scheduled school holidays or vacations (see “absences” below). If the tenth falls on a Saturday or Sunday, your payment must be received by the office or postmarked by 5:00 p.m. of the Friday before in order not to be deemed late. All checks should be made payable to Centerpointe Christian Preschool. Cash payments will be receipted. All tuition payments should be mailed or personally delivered to the office. Payments brought to the office should be deposited in the designated box. Please note that the school cannot be responsible for tuition that is carried by a student.

RETURNED CHECKS

Tuition payment checks that are returned by the bank because of insufficient funds will be assessed a service charge of $25. A returned check will be considered nonpayment and the $35 late fee will be assessed to all such accounts not paid by the 10th.

OVERTIME CHARGES

An additional fee of $3/minute will be charged if a child is not picked up by the end of their program. (1:00 pm for AM, and 6:00 for FT). If a parent needs a morning child to remain through the afternoon, please submit a request through the office by the Monday of the week that extra time is required. If space is available, your child will be accommodated and an additional half-day charge of $35.00 per day will be assessed. In consideration of our teachers who have other commitments and responsibilities after the school closes for the day, all children must be picked up by 6:00 p.m. If a child is picked up after 6:00 p.m. a late charge of $3 per child for each minute after 6:00 pm will be assessed, payable upon receipt of the billing.

ABSENCES

Because the tuition contract is made by the parents to guarantee a place in the school for their child, and is the basis for providing personnel to staff the school, no reduction of tuition for any absences due to illness or vacations can be given. In case of an extended illness (hospitalization), a conference with the Director should be arranged.

VACATION BENEFIT FOR LONG TERM STUDENTS

Once a full-time child has completed 12 full months of continuous enrollment, a family will be given upon request a 1 week tuition credit for vacation.

NAP KITS FOR FULL-DAY STUDENTS

All full-day students must purchase a Nap Kit consisting of a pillow, blanket, and bag from Centerpointe Christian Preschool for $30.00 prior to the first day of class.

WITHDRAWAL/REFUND POLICY

Official withdrawal must be made by completing a withdrawal form in the school office two weeks before the date of withdrawal. If a student withdraws from the 1st to the 15th of the month a one-half month refund of that month’s tuition will be given. No refunds will be given if a student withdraws from the 16th to the 31stof that month. One-half month’s tuition will be charged in addition to the above if two-week notice is not given when a student withdraws. Registration and Supply Fees are non-refundable. No exceptions will be made. If your child has withdrawn from Centerpointe Christian Preschool and you wish to re-register them at a later date, an additional registration fee will be required.

DISMISSAL

In order to ensure a proper fit with the school, each child is accepted into the program on a trial basis for the first ten days of his/her attendance. During this time, should it become evident to the preschool staff that the child should not continue in the program, the child can be dismissed without prior notice and any unused prepaid fees will be refunded within 48 hours. In cases involving dismissal after the ten day period, the school will give at least one-week notice and will refund any unused prepaid fees within 48 hours. A child may be dismissed from the program if it is determined that they no longer benefit from attendance due to:

a. Child’s inability to separate from parent and it is agreed by parent and school that the child is not yet ready for a school setting;

b. Child is unable to control aggressive actions to other children or staff personnel;

c. All children must be able, at appropriate age level, to reasonably manage and control all body fluids (i.e., urine, bowel movements, runny nose, etc.);

d. Parent or guardian has not cooperated with the Preschool regarding the child’s discipline needs;

e. Parent or guardian has not paid the agreed upon fee or has been late paying the fee more than twice in a six month period.

f. Repeated late pick-up after 6:00 pm may result in dismissal.

CENTERPOINTE PRESCHOOL HANDBOOK

We acknowledge that we have received and read a copy of the Parent’s Handbook and we agree to all of the Preschool’s operating policies and procedures as described therein. We understand that Centerpointe Christian Preschool is a NUT FREE SCHOOL. We will help keep all students safe by providing lunches that do not contain any type of tree nuts including: cashews, pecans, macadamia nuts, almonds, pine nuts, walnuts, nut oil, nut paste, pesto, nougat, pralines & nut butters.

AGREEMENT

We as parents/guardians of the student applicant, do sincerely give our pledge to this entire agreement. We understand that failure of the parents or child(ren) to comply with the established regulations and discipline, parental commitment, or failure to meet financial obligations will forfeit the student’s privilege of attending Centerpointe Christian Preschool. We also understand the preschool reserves the right to raise tuition fees and/or modify any of the conditions of this agreement upon 30 days written notice to the parent or guardian.
